Packing against a verbal instruction with nothing recorded. The part leaves, the invoice does not, and the money is late before anybody knows.

Written down for the first time in August 2026. It had lived entirely in one person's head and in a spreadsheet, which is why a dispatch made on a day he was out could take three days to reach an invoice.

Nothing waits for an invoice because dispatch has not been written up.
